New students will need to provide the office with the following items/paperwork:

• Student Enrollment Checklist

• Student Enrollment Form

• Health Inventory Form

• Residency Form and Documentation

• Admission and Withdrawal Prior Discipline Form

• Student Technology Agreement

• Immunization records

• Copy of Birth Certificate

• Free or Reduced Lunch Program Application (if applicable)
